Telehealth intervention in the continuous nursing of patients with lung transplantation:a scoping review

Objective To identify the specific interventions,content elements and application effects of telehealth,so as to provide the guidance for future research. Methods The literature was retrieved from PubMed,Embase,Cochrane Library,Web of Science,CNKI,Wanfang,and CBM databases. The included studies were summarized and analyzed. The search time was from the establishment of the databases to January 5th,2023. Results A total of 29 articles were included. The intervention forms included application,portable electronic device,network communication software,and website. The content elements included remote monitoring,education,rehabilitation,outpatient,psychological and cognitive intervention. The outcome types were mainly physiological indicators,psychological indicators,self-management,clinically outcome,and feasibility. Conclusion The main forms of continuous nursing are mobile applications and portable electronic devices. Comprehensive content and rich functions of devices are more favored by patients. Telehealth intervention is feasible,safe,and effective. Medical workers can further broaden the service of continuous nursing for patients with lung transplantation by remote technology. In the future,studies on telehealth intervention are still needed to promote the health of patients and improve the outcomes of patients.

Exercise intervention in the rehabilitation of patients with lung transplantation:a scoping review

Objective To identify the specific intervention content,methods and effects of exercises,so as to provide the guidance for future research.Methods A literature search was performed in the CNKI,Wanfang,CBM,the Cochrane Library,PubMed and Embase to identify relevant studies. The included studies were summarized and analyzed.Results Totally 25 studies were included,of which 9 were randomized controlled trials and 16 were quasi-experimental studies. The results showed that exercise methods can be divided into 3 categories,namely aerobic exercise,resistance exercise and flexibility exercise. The intensity of exercise was mostly low to moderate. The frequency of exercise was mostly 3~5 times per week,lasting for 30-60 minutes per session. The results showed that exercises had positive effects on respiratory function,exercise ability,quality of life and mental health of lung transplant patients.Conclusion Exercise intervention is holistic,dynamic and comprehensive,with a positive effect on lung transplant patients. Future research should focus on exploring the best exercise intervention strategies,formulating standardized and unified evaluation standards,and formulating scientific,applicable and standardized exercise training programs for lung transplant patients based on the condition and physical function of lung transplant patients.

Objective To explore the real experience of mild cognitive impairment patients with memory decline,and to provide references for clinical staff to carry out follow-up medical nursing intervention. Methods Using objective sampling method,9 patients with mild cognitive impairment in the Department of Neurology of a tertiary hospital in Beijing from May to June 2021 were selected as the research subjects for semi-structured interviews,and the data were analyzed by Colaizzi 7-step analysis method in phenomenological research. Results The real experience of memory loss in patients with mild cognitive impairment can be summarized into 3 themes:disease perception of memory loss(attitude of contempt and denial,introspection of reflection and traceability,and change of interactive relationship);emotional experience of memory decline(generation of anxiety and fear,reduction of self-efficacy,formation of self enlightening attitude,expectation of facing the future);coping strategies for memory loss (lifestyle coping,physical and mental activity coping,future planning coping). Conclusion We should strengthen the popularization of medical knowledge about memory loss in patients with mild cognitive impairment,pay attention to their emotional needs,give psychological counseling,family support and social support,and carry out personalized cognitive intervention to promote their cognitive improvement and return to family and society.

Sinicization of the Dementia Quality of Life Scale for Older Family Carers of Patients and the test of its reliability and validity

Objective To test the reliability and validity of Dementia Quality of Life Scale for Older Family Carers(DQoL-OC) in Chinese version. Methods According to the modified version of Brislin retranslation model,the original DQoL-OC was translated into Chinese version. The convenience sampling method was used to survey 242 older family caregivers from outpatient and inpatient patients. 50 caregivers of them were randomly selected to retest after 30 days,and the data was collected to analyze the reliability and validity. Results The Chinese version of DQoL-OC contains 22 items with one dimension,which is consistent with the original scale. The I-CVI was 0.83~1.00;the S-CVI/UA was 0.82;the S-CVI/Ave was 0.91;the structure validity χ2/DF was 2.33;the approximate error mean square root(RM-SEA) was 0.07;the fitting goodness index(GFI) was 0.92,and Tucker-Lewis coefficient(TLI)was 0.92.The internal consistency index Cronbach’s α coefficient was 0.92;the test-retest reliability was 0.81. Conclusion The Chinese version of DQoL-OC has good reliability and validity,which can be used to evaluate the quality of life of older family caregivers of dementia patients. It can provide corresponding bases for researchers to make targeted intervention strategies for the older family caregivers.

Care needs in lung transplant recipients at home:a meta-synthesis of qualitative research

Objective To systematically review and synthesize qualitative studies on the care needs of lung transplant recipients at home and to provide evidence-based guidance for the development of educational programs and the implementation of continuous care. Methods We searched databases including PubMed,Embase,Web of Science,Cochrane Library,CINAHL,ProQuest,CNKI,Wanfang Data,VIP and CBM from inception to August 2020,to collect qualitative studies exploring the care needs of lung transplant recipients at home. The Joanna Briggs Institute Critical Appraisal Tool for qualitative studies in Australia(2016 edition) was used to evaluate the quality of included studies. The results were synthesized by integrating methods. Results A total of 11 studies were included,with 60 complete research findings grouped into 11 categories by their similarities. 4 integrated findings were synthesized,namely informational needs,physiological needs,psychological and spiritual needs,and societal needs. Conclusion The care needs of lung transplant recipients at home were diverse and special. Transplantation teams should pay more attention to the care needs of this population,and provide education,monitoring,pulmonary rehabilitation,and social support for discharged recipients,in order to improve the self-management ability and their quality of life.

Construction of a nursing quality index system for cognitive training nursing clinics

Objective To construct a quality index system for cognitive training nursing clinics in line with China’s national conditions,and to provide references for related practitioners. Methods Based on the literature research,theoretical analysis,and clinical investigation,a draft on indexes was constructed. Using Delphi’s expert consultation method,16 experts were selected and 2 rounds of consultations were conducted to establish a nursing quality index system for cognitive training nursing clinics. Results The recovery and effective rates of 2 rounds of consultation documents were 100%,and the authority coefficient of the second round of consulting experts was 0.836. Expert opinion coordination coefficients of the 3-level indexes were 0.755,0.813,0.929 respectively(P<0.05). The final quality index system includes 3 first-level indicators(structure quality,process quality and outcome quality),10 second-level indicators,and 34 third-level indicators. Conclusion The quality index system of cognitive training nursing clinics constructed in this study is scientific and practical,and it can provide references for comprehensive evaluation and standardized management of cognitive training nursing clinics.

Objective To formulate an expert consensus on diaper dermatitis nursing care for infants and toddlers,and to standardize the clinical nursing practice. Methods Literature searching/reviews and expert discussion meetings were used to formulate the main contents of the consensus. The consensus was then revised by 2 rounds of Delphi expert consultations and expert meetings. Results The final expert consensus on diaper dermatitis nursing care for infants and toddlers included 3 aspects of diaper dermatitis nursing care(evaluation,prevention/nursing care,treatment of infection),and a total of 31 items. Conclusion Items in expert consensus on diaper dermatitis nursing care for infants and toddlers are concise and easy to implement,and the consensus is helpful to guide clinical practice of diaper dermatitis nursing care.

Analysis of health status and its influencing factors of community residents based on the intelligent health monitoring system

Objective To investigate the health status of community residents and to analyze related influencing factors. Methods A descriptive cross-sectional study was conducted to obtain data of 4 311 community residents in China from April to May,2019. Intelligent health monitoring system,including their general characteristics,health behavior,health status,results of generalized anxiety disorder scale-7(GAD-7) and patient health questionnair-9(PHQ-9),was used as the suvery tool. Descriptive statistics,chi square test and logistic regression were used for data analysis. Results The average age of the participants was(29.25±11.88) years old,and the prevalence of chronic diseases was 13.8%. There were 1 480(34.3%) patients with anxiety and 1 620(37.6%) with depression. The smoking rate and drinking rate were 12.2% and 32.3% respectively. Age,education level,being married,meal regularity,additional meal in the morning,eating dinner,meat-based diet,salty diet,other special diet,drinking alcohol,taking vigorous exercise,anxiety and depression were the influencing factors of chronic diseases. Conclusion The health condition of community residents was generally good,while the symptoms of anxiety and depression were common,which implies that much attention should be paid on the emotional health. The risk factors of chronic diseases were old age,being married,irregular meal,meat-based diet,salty diet,other special diet,drinking alcohol,no vigorious exercise,depression and anxiety. These findings can provide suggestions for the community to develop interventions to promote residents’ health.

Effects of psychological contract intervention on subjective well-being promotion among new pediatric nurses

Objective To investigate the effects of intervention based on the theory of psychological contract on subjective well-being among new pediatric nurses. Methods A total of 112 newly recruited nurses from 2016 to 2017 were selected to carry out training on subjective well-being in a pediatric tertiary hospital in Shanghai. An one-year training program based on the theory of psychological contract was developed,including comprehensive pre-employment training,individualized training mechanism and incentives,timely and effective communication model,to provide multiple supports and incentives on life,work,feeling and profession. The General Well-Being Schedule and psychological contract scale were used to evaluate the effects of the intervention. Results After intervention,psychological contract fulfillment increased gradually,and the score of subjective well-being was higher than before(P<0.05). Conclusion Nurse subjective well-being promotion intervention based on psychological contract can enhance subjective well-being among new pediatric nurses,stabilize pediatric nursing team,but it needs to be further improved and to become a continuous program.
